Caregiver Support Project 2.0 – Walk Together

Ms Mak, the caregiver in her eighties, has shared a lifetime of love with her husband. Since her husband diagnosis with dementia, she has shouldered the caregiving responsibilities ever after. In recent years, his health, cognitive abilities and mobility have deteriorated, increasing his reliance on Ms Mak, which makes her lack of personal time and intensifies the pressures of care. 


The “Caregiver Support Model (CSM)” is a proven and locally validated assessment tool which provides an evidence-based and comprehensive examination of the 5 main needs and 7 essential resources for caregivers. Our social workers and caregivers to participate in decision-making on personalized intervention plans. It enhances the effectiveness of matching suitable and personalized services and supports for their needs.


After joining the Caregiver Support Project 2.0, Ms Mak recognized her own needs and strengths, learning to nurture her physical and mental health while effectively utilizing community resources. This brought positive changes in her life and family. Moreover, seed social workers from Methodist Centre have established strong relationships of mutual trust with caregivers, providing more professional and comprehensive intervention support to walk alongside them on the caregiving journey.
